Per your request, I have compiled all incident reports involving our forces engaging the demonic entity known as Vex. The threat this being poses cannot be overstated, as evidenced by the severe casualties and property damage inflicted in multiple encounters thus far.

The first confirmed clash with Vex occurred in the town of Ares on October 14th during a widespread interdimensional portal incursion event. Eyewitness accounts describe Vex suddenly appearing through a spatial tear before unleashing swarms of gnashing monstrosities on the populace. Attributing forces were quickly overrun, trying to evacuate civilians, resulting in 58 soldiers killed in action.

After this tragedy, Vex remained dormant for two years before resurfacing. Military officials suspect he had infiltrated at high levels during this period. On August 3rd, Vex launched a devastating surprise attack on Fort Bulwark near a major portal nexus. Using destructive psychic abilities, he disabled our automated defenses and compromised the base's interdimensional shields. A swarm of hostile non-humanoid entities poured through the breached defenses, slaughtering 342 personnel and civilians combined before being contained at great cost.

Most recently, we have confirmed Vex was directly involved in the catastrophic events of the June 15th Cerberus Incursion. Eyewitness reports place Vex at the scene coordinating portals to enable overwhelming demonic forces to invade from his domain. Our regular troops were unable to contain the incursion until an experimental Magi Tech unit arrived and successfully captured Vex, attenuating the threat. However, Vex's actions prior to capture resulted in over 100 killed in action from this latest assault.

In summary, Vex represents an adaptive, cunning foe wielding both tremendous metaphysical power and strategic intellect. Current countermeasures have proven inadequate across three major encounters. Developing effective defenses against this demonic threat should be the Army's highest priority.
