When Sven wakes up, he finds himself lying on a hammock made of hide with an oxygen mask on his face. Soon Sven meets the guru of the foggy settlement, who turns out to be Emma's twin sister, Emma. She is barefoot and dressed in bikini-like animal skins, with a deer-antlered headband and drone details on her head. Emma could not fully realize that she had met her sister again in such a position. She was shocked by her sister's appearance, but promised that she would find a way to completely suppress the virus in her body.
During a general conversation around a large fire, all those gathered mourn Victor. Emma admits that she was the one who piloted the hacked reconnaissance drones that delivered the weapons to Sven at a critical moment. Sven informs him of Jacob's death, to which Emma replies that this is a great chance to bring the island people together again. Emma regrets not being able to bring everyone here earlier, as the concentration of decontamination gas (fog) was not enough to apprehend Lewis, who is a super mutant capable of producing and taming virus cells.
The entire group of islands is shrouded in a similar fog. Fog serves as a barrier to islands and the rest of the world, preventing carriers of the virus from escaping from isolated territory. The community on the islands began to form during the First World War from soldiers and army personnel. These people were found to have traces of a new virus, which instantly killed their entire environment. Before death, all the patients claimed that they saw a certain female entity staring at them from the outside. Finding that the members of the group are unable to kill each other, they are placed on an isolated island in the North Sea off the coast of northern Scotland, as it was here that the first samples of the virus were studied.
The Observatory Corporation organizes constant surveillance on the islands. At present, several generations have already changed in the community. Observatores sought to preserve the natural evolutionary process of social development in the community. They find that every time a generation changes as a result of the death of previous hosts, the virus mutates, so they try to prevent the complete extinction of the community by delivering supplies (weapons, food, clothing, etc.) to them at critical moments using drones.
Also, by observing the community, the organization collects their genetic material, discovering a new kind of perfect table cells, with the help of which the organization has learned to grow modified organs. The organization extracted genetic material with the help of drones from the bodies of dead members of the community. The Observatores feared that with the complete extermination of the community, they would lose the only available samples and be left completely unarmed in the next likely outbreaks of the virus.
Emma admits that for a long time she was the one who delivered supplies to the most vulnerable part of the community, which may have been the reason for her split, as the guru of the abstract mother frowns on accepting any help from mechanical birds.
Upon learning of Lewis's plans to wipe out all the patients and inhabitants of the islands, Emma hurried to leave Northern Pelais to counter it.
In the new settlement, she was greeted as a true guru of the abstract mother for her talent in flying drones and helping in the fight against Jacob's settlement.
There, she was informed of Anne's pregnancy and Lewis's sinister plans for her unborn child. Emma realized that this child was the true target of Lewis and the Observatores, because thanks to him they would be able to create a full-fledged vaccine against the virus, which would eventually lead to the death of the entire population of the island. She vowed to protect their child from Lewis at all costs.
One evening, when Emma was suffering from severe withdrawal without serum, she was brought to Emma's chambers. She promised to heal her sister from addiction and return her to her former appearance with the help of her latest development. However, when she was given an injection, the drug did not have the desired effect.
Emma gave her one of the last remaining vials of serum to relieve the withdrawal symptoms and struck up a heartfelt conversation with her sister. She admitted that she could not take her with her for fear of endangering her, so she arranged her disappearance in order to escape. Emmy asked her not to make excuses, assuring that she was just glad to meet her sister. Remembering some moments from their early childhood that Emma did not remember, the girl touched her. Emma felt regret for her sister's fate.
In one of Northern Pelace's offices, Lewis had sex with a therapist, hoping in vain to achieve the desired result. He asked Teresa where her former arousal was and why she did not give him the opportunity to be reborn. Receiving no answer, Lewis went to the mirror and saw a piece of flesh hanging from his aged, wrinkled face. His appearance and age were identical to those of Gerhard Grechem from the photograph that Sven had seen on the computer in the staff room.
Exposing his torso, covered with scars and stitches from numerous operations, Lewis loudly asked Teresa what was wrong with him. After another unsuccessful attempt at coitus, Teresa wondered why Gerhard did not love her as a woman and wife. She recalled that Gerhard grew cold towards her after he made a breakthrough in his research. The woman also passed on complaints from Rudy, who drew attention to the fact that his father regularly avoided him.
Lewis coldly replied to Theresa that her illness had changed their family a lot, putting it back together piece by piece. These words upset Teresa, and she refused to continue copulation. As he left, Lewis once again reminded her not to leave Northern Pelijs under any circumstances. Theresa then asked Lewis to finally bring Lily to her, to which the man replied with a cold silence.

About a month has passed since Sven lived in Emma's settlement. With constant hunting, the man tried to drown out the severe withdrawal from the serum, the reserves of which were already running out. Also, thanks to the drone, Jacob's former completely devastated settlement, led by Beatrice, came to Emma's community.
Emma considered it a great achievement that the island people were united again, but more than half of the settlement perished on the way here, fighting their way through the fog teeming with hordes of fiends. Beatrice herself gained the same ability to regenerate as Sven, but became dependent on the serum. Sven and Beatrice found an effective way to numb bouts of severe withdrawal by driving a nail into their palms.
Anna was already at a late stage of pregnancy and therefore was unable to excrete serum from her blood. Sven took Emmy's isolation from the rest of the community hard. The girl fell into delirium and became especially aggressive due to withdrawal without doses of serum. Many members of the community saw her as a dangerous fiend.
Among the supporters of Emmy's isolation were a large number of people who came from Jacob's former community, who had suffered heavy losses due to attacks by fiends. In order to prevent a split, Emma had to make the difficult decision of isolating the girl by giving her small doses of recycled serum.
Returning to the community with two deer shot, Sven visited Anna, who complained of constant violent fights. At a community meeting, Emma announced alarming news - the amount of fog surrounding the settlement and restraining the fiends has been greatly reduced.
The girl said that the remaining serum and all its substitutes were used to synthesize the antiviral fog she invented to contain the fiends. Emma announced the strengthening of the perimeter of the settlement, calling for the formation of security squads to eliminate random fiends that could attract others. Left alone with Sven, Emma admitted her failure to create a serum substitute, warning that it could be dangerous for Emmy's condition.
Suffering from insomnia, Sven turned to Anna, but instead he saw the silhouette of a naked woman, who ran her hand over his face, asking at what point he would fall apart. When Sven woke up, he woke Anna up with his scream. They had a heartfelt conversation, during which Anna asked Sven to promise that he would take care of their child. Sven calmed Anna down, promising that he would comply with her request.

During the next evening service, Sven watched as Emma offered a prayer to her abstract mother. He drew attention to the fact that not all members of the community were present at this ritual.
After the conclusion of the mass, Sven asked Emma how such a convinced atheist as she could believe in the existence of a deity. Emma, approaching the image of the mother, created from ears of hay and animal skins, declares with full confidence that this abstract figure is a chthonic living embodiment of the virus, endowed with intelligence and striving to create a new person who will live in harmony with nature.
According to Emma, the incarnation was created by nature itself and endowed with intelligence for one purpose - to re-breed the human species so that it lives in unity with the environment, while eliminating modern man, who, unfortunately, destroys nature around him. Observing the incarnation, Emma comes to the conclusion that only carriers of the virus can come into contact with him.

During another nightmare, Sven finds himself on the streets of a large metropolis, far from crowds. As soon as it approaches people, they turn to dust. In the midst of the crowd, he meets all the people dear to him - Anna, Victor, Beatrice and others, but when he approaches them, he also turns them to dust. On the other side of the street, Sven sees the silhouette of a naked woman walking towards him, dissolving all the people he meets. When the silhouette is very close to Sven, he screams heartbreakingly.
When Sven wakes up, he realizes that the scream belongs to Anna, who has gone into labor. She is taken to a two-story mansion located at the end of a wheat field. At this time, Emma detects a minimum concentration of containment fog, and the drones watch as the horde of fiends, as if on bait, begins to move towards the settlement.
Realizing the criticality of the situation, Emma organizes the defense of the settlement, and Beatrice decides to personally give birth. Despite heavy losses, the defenders are unable to hold back the horde. Once in the settlement, the fiends head towards the mansion, killing everyone in their path.
Retreating from the fiends, Emma frees her sister, who has finally gone wild from withdrawal, who immediately tears apart all the monsters she comes across. The survivors organize the last line of defense on the first floor of the mansion, shooting numerous fiends crossing the wheat field. To overcome another withdrawal attack and focus on defense, Sven smashes his head against the windowsill.
At one point, the fiends even break into the mansion, where they are dealt with by Emmy. Having used up almost all the ammunition and with heavy losses, at dawn the defenders still manage to repel the horde and hear the cry of a newborn girl with a reddish tint to her eyes, reminiscent of the color of serum.
Holding the newborn, Beatrice tearfully reports that Anna died, unable to withstand the difficult birth. Among the survivors, there are many supporters of the complete eradication of fiends from their ranks. They threaten Emma that they will kill her sister, the newborn, Sven, and Beatrice, as they are fiends and threaten their safety.
Despite all the persuasion, the attackers shoot Emma, provoking Emmy into a fight. During the scuffle, Sven says goodbye to Anna for the last time, reminding himself of his promise. Taking the child, Sven throws himself out of the mansion window.
While hiding in a ruined settlement, Sven manages to kill several intruders pursuing him. Suddenly he hears a familiar frantic roar. From the sidelines, Sven watches as a heart-shaped carcass bursts into the settlement, destroying and killing everyone in its path.
After tracking down Sven, the carcass immediately heads towards him, shattering many of his bones and organs with a powerful blow. Trying to protect a man with a child, Emmy engages in a fight with a carcass. Sven gives the child to Beatrice, telling him to protect him from Lewis at all costs.
Next, Lewis, who is very old and disfigured by past traumas, appears in the settlement. After killing the remaining settlers, he makes his way to the mansion, where he sees Emma bleeding to death.
Dying in Lewis's arms, the girl sees in him the exact image of her father, rejoicing that he has finally come for his daughters. Grief-stricken, Lewis closes the girl's eyes and goes in search of the newborn.
Meanwhile, Emmy manages to immobilize the carcass, causing many injuries to it. Seeing her father in Lewis, a joyful Emmy misses a critical blow from the carcass, which knocks her into the mansion to her sister's body. Before dying from numerous fractures and injuries, with tears in her eyes, Emmy communicates with her sister's corpse, rejoicing that her father still came for them. Lewis soon catches up with Beatrice.
With a blow of his hand, he blows off the girl's head, picking up a newborn. As he approaches the bleeding carcass, howling in pain, he hears the familiar scream of Theresa mourning for Emma. She accuses Lewis of lying, manipulating Rudy, and the death of their daughter.
Noticing a half-dead Sven, Lewis puts the child next to him, heading back to Theresa. In a cold tone, Lewis admits that the woman is right. This infuriates Teresa. She asks Rudy to kill Lewis. The carcass then unleashes several sudden blows, one of which literally knocks some of Lewis's insides out, but does not kill him.
With a deft blow of the blade formed from his hand, Lewis pierces the heart of the carcass. Approaching the tearful Teresa, he informs her that he has saved their son from suffering and now it is her turn. After killing the woman with a swing of the blade, Lewis throws a vial of serum at Sven, who, bouncing, falls a few meters away from him. Sharing his opinion that Sven will be a good father, Lewis drops dead from his injuries, next to the bodies of Theresa, Toucha, Emma and Emmy.
Sven, overcoming the pain, crawls up to the child and carefully takes him in his arms. Slowly moving towards the coveted ampoule, he realizes with horror that there is no one left on the island except the two of them.
When Sven was about to grab the ampoule, suddenly his hand was stopped by a woman's naked foot, breaking the ampoule into pieces. Sven seemed to be in a trance, raising his head, he saw the silhouette of a naked woman, around whom many tentacles writhed, repeating the shape of the DNA helix. This silhouette seemed so real to him that he realized that in front of him was the embodiment of an abstract mother.

Sven heard the voice of the incarnation in his head, asking him to look around and see the consequences that her love for children had brought. Toxic barrier fog has poisoned local soils, making them unsuitable for cultivation. The new children were supposed to be closer to their mother, but they were born of proud brothers and sisters, having received free will from them.
By granting them retribution, the abstract mother hoped that Sven would clear the desecrated fields so that he could rebuild the garden in their place. The Incarnation realized that no matter what her children were, they would always have a will, because they were part of her flesh and blood. This will awakens in them selfishness and pride, for the sake of which they are ready to destroy any garden. But it is the will that makes the offspring of her incarnation children. Having given her child freedom, the mother gave him freedom, with which he can choose his own future.
Looking at Sven with a cold gaze, the incarnation asked the question: how could her selfish child, like a mother, create an incarnation of herself from parts of herself, ready to die for another life? Taking the child from Sven, the incarnation thanked the man for proving to her that her vicious child was still worthy of the future, despite his vicious will, which was capable of more than destruction.
Unable to resist, Sven stared blankly at the incarnation into the distance. When he woke up from his trance and looked around, he saw only dead bodies and destruction, realizing that he had lost a child. In utter despair, Sven falls to the ground, dying of internal and external bleeding.
Awakened by the sound of a bell in the sunlit chamber of Northern Peleis, Sven looked around. Two twin nurses stood by the bed with a tray full of pancakes with honey and butter. Introducing themselves as Emma and Emmy, the girls said that Sven had already missed breakfast, but by special order of the head of Northern Peleys, Miss Dominica, the staff was obliged to feed, change clothes and bring the man to her private office.
After breakfast, Sven, accompanied by the twins, went to Miss Dominica's office. On their way through the various Northern Peleis complexes, the twins greeted oncoming staff and patients. Among them, Sven constantly saw recognizable faces and heard familiar names: the jolly Victor, playing table tennis in the main hall with the grumpy old Japanese Sshiru Koto; a young girl Anna, helping a gardener in the courtyard of the institution; Beatrice, the maid, dusting out patients' rooms.
Finding himself at Dominika's reception, Sven immediately noticed her eyes of a reddish hue. Dominica looked at the man for a few minutes, as if comparing him to someone, after which she greeted Northern Peleys into the ranks of patients, promising to improve his condition. A sudden phone call interrupted the conversation between Dominika and Sven.
Leaving the office, Sven heard Dominika agree with her interlocutor on the phone, that the new patient is similar to a certain Mr. J. An unknown interlocutor suggested that Dominika move on to the next part of the operation, since all the witnesses of the incarnation have already been provided to the girl. After a short pause, Dominika asked her interlocutor to wait for her signal about the beginning of the next stage. Hanging up the phone and hearing a creak outside the door, Dominika realized that her conversation had been overheard.
