In the autumn forest at night, by the fire, several men in shabby clothes listen to the story of their comrade. In the distant past, when consciousness did not yet exist, a woman was looking forward to the birth of her child. When this happened, she was overwhelmed with sincere love for her baby, who had become a pure and pure being.
However, over time, a will similar to that of his mother began to awaken in the child. This will was imperfect, but the mother could not find the strength to take it away from her child. The story is interrupted when one of the men asks to focus on the search for Carlos's daughter, who disappeared a few days ago. Suddenly, suspicious moans are heard from the forest thicket. After ordering the others to take their weapons, the narrator and the group slowly approach the brush, where they find a naked girl lying unconscious. Wondering where it came from, men cover it with their jackets.
Suddenly, the girl's body begins to transform rapidly, overgrown with many deformed limbs. Turning into a monster, she tears out the arm of one of the men and throws herself at the other. However, a well-aimed shot from the shotgun shatters into pieces, and the monster's head falls to the ground.
After recovering from the shock, the men realize that this is the first time they have seen a fiend in human form. One of them suggests that what happened confirms the theory of their dead comrade that all fiends come to them from those who launch mechanical birds. Examining the remains of the monster's head, they pay attention to the eye, in the eye socket of which they see a lens from the camera watching them right now.
The beginning of the two thousand years (approximately 2002–2003). Sven is a young, thin man in his early 30s, with slight stubble on his face (possibly glasses), working as an engineer at a construction site. One day, he falls into a trench and is seriously injured, losing consciousness. Waking up after an unknown amount of time, Sven is confused to find himself in a straitjacket on a bed in what appears to be a mental hospital.
He is approached by two twin nurses named Emma and Emmy, greeting him on behalf of the entire Northern Pelais facility and its caretaker, Gerhard Greckham. The girls dress Sven in the general uniform of the patient and bring him to the general building of the institution, making him feel at home. They indicate that it is possible to leave the territory of a completely isolated complex only after a complete recovery and a personal order from the head of the institution.
Remembering nothing but the moment of falling into the trench, the man begins to explore his whereabouts, trying to remember why and for what purpose he ended up here. To his surprise, everyone he meets immediately recognizes him, claiming that he is a long-time patient of Northern Peleys, undergoing experimental stem cell treatments for some little-studied disease. New acquaintances explain that a couple of days ago, on the orders of Gerhard Greckham, he was transferred from the general building to the closed department B8, after which he was transferred back in an unconscious state. Interlocutors say that the transfer of a patient from B8 is very rare, because almost all patients who are there never return.
Sven is even more alarmed by the medical staff of the institution, who avoided communicating with patients in every possible way, hiding their faces behind medical glasses and masks. One day, he is tormented by a sudden toothache, and he is examined by a dentist, who notices that he has a wisdom tooth. After extracting the tooth without any painkiller, the dentist urges Sven to contact him again if he has any complaints. A few days later, Sven is again seen by the dentist, who again extracts his tooth in the same place without any anesthesia. The dentist informs Sven that he has all the indications for a course of taking the serum. He summons Emma and Emmy, handing them Sven's patient card with a referral for a course of serum.
After the examination, the twin sisters inject Sven with an unknown dark red serum, mentioning his wife, who, according to the records at the checkpoint, visited him for the first few months after arriving at the facility.
Soon, while walking in the courtyard of the institution, enjoying the autumn views of yellow foliage and thinking about what is happening, Sven hears the confession of one of the patients, who claimed that during the ultrasound he noticed the presence of repetitive organs on his pictures, perceiving it as a hallucination from local drugs. The next night, trying to sleep, Sven is tormented by nightmares, in which a bubbling noise is heard from the depths of the ocean. Among the incomprehensible rustles and screams, a whisper gradually becomes heard, persuading Sven to believe only him.
After Sven shared his regular nightmares with the twins, they took him to see a psychiatrist, an attractive woman with glasses who looked to be about 35 years old. From her, he learned that all patients at Northern Palace Hospital faced the same problems.
Noticing the woman's rounded belly, Sven assumed that she was expecting a child. Before leaving, he noticed a badge on her chest that had the name "Teresa" written on it. Sven politely thanked her for the reception and closed the door behind him.
One evening, Emma and Emmy, dressed in nightgowns, were getting ready for bed in their room. When Emma was already in bed, Emmy hovered over her and began a conversation. Both girls hoped that their father would pay more attention to them, as he did in childhood. Emmy suggested that the reason for such a change in the father could be the death of their mother during the birth of twins. Emmy promised her sister that they would always be together, and received the same response from her.
Soon, Sven's visits to the therapist became daily. The twins took him to Theresa every morning, who questioned him about increasingly strange things, such as his dreams and preferences for women. With each passing day, it became more and more difficult for Sven to perceive what was happening: reality mixed with memories from the past, making him doubt his own sanity.

One rainy night, Sven woke up in his room to a loud noise coming from the corridors of the hospital. He felt unbearable pain and took an ampoule of dark red liquid lying on the floor, immediately injecting it into himself. When Sven came to his senses, he went out into the corridor and was horrified to see many dead patients. As he walked through the dark corridors of Northern Peleis, he noticed a young woman sitting motionless. Sven recognized her as a familiar nurse. As he came closer, an extra hand suddenly appeared from the woman's chest, grabbed a sharp metal rod and plunged it right into Sven's heart. In terror and unbearable pain, Sven fled, attracting the attention of aggressive patients with extra limbs and appendages all over their bodies. He managed to hide in the sewer, but he knew he should be dead by now.
Sven took out the bloody rod, feeling it literally come out of his heart. Realizing his position, he realized that he was in the isolated block B8. He remembered his wife and decided to get out of Northern Pelace. In the locker room for the staff, he found clothes of his size and changed. Sven went out into the corridors of the gated complex again. He noticed the lighted staff room and headed there. Among the many corpses, Sven found a computer with the results of a molecular study of the "perfect quantum virus".
He collapsed the article and saw a photo of all the staff in front of the complex building. Among the crowd of people, Sven noticed two little girls, whom he recognized as his grown-up twin sisters.
With the help of improvised means, Sven tried to fight his way to the general building of the hospital, fighting off the changed, distraught and disfigured patients of Northern Peleis. The attackers had extra limbs (arms, legs, part of the torso with arms and head), as well as body parts of their conjoined twins, which formed on their torsos.
In addition, the changed had sharp mood swings: in a minute, you could see terrible laughter or crying on their faces. Some of the particularly dangerous and fast altered ones had multiple faces on different parts of their bodies.
Throughout the journey, Sven felt a strong craving for the dark red serum and searched for it in the offices. Exploring abandoned rooms, he finds many fragments of research by a certain corporation called Observatores, which document the presence of repetitive organs in Northern Peleis's patients. Sven recalls that on many objects, including ampoules of serum, documents, equipment, etc., he saw a logo with the word "Observatory". Among the many archive folders, Sven finds a report on the effect of red serum on the suppression of "retaliation" cells in the bodies of "objects", as well as the fact that it causes them to become addicted to ensure the constant suppression of virus cells.

At some point, Sven stumbles upon the body of one of the orderlies and finds bandages to bandage his chest wound. It continues its way down the main corridor to the common body, but is attacked by a giant carcass consisting of many deformed limbs and torsos. The carcass resembled a human heart with giant front legs that allowed it to move at incredible speeds. With her attacks, she breaks half of the corridor, and Sven and the monster fall to the underground level of the complex. When Sven wakes up, he realizes that the only way out is blocked. He continues to move through the catacombs, looking for a way out, and finds a bleeding old Asian man in the form of a patient wearing a Shiru Koto badge. The old man in delirium tells his wife that he drank his liver, but thanks to the orderlies, he now has a new one. Seeing Sven, the old man calls him "another part of the orderly designer" and asks him to be quiet, because soon "they" may appear. The old man does not answer Sven's questions, but insistently asks him not to look at "them".
The old man laughs viciously and gives Sven a security baton with a flashlight, which he took from the corpse of Bobby, a guard known for bullying patients in this block. The old man, panting, says: "Illness affects everyone in different ways: it changes us and kills them." A disfigured hand emerges from his mouth, tearing him apart.
In a large, dark room with many columns, Sven is attacked by a group of fast humanoid creatures - female silhouettes without eyes and many arms growing out of their backs. They try to pluck out his eyes. Remembering the old man's words, Sven pulls the hoodie over his head, and the silhouettes lose sight of him. He breaks out of the trap and, passing through intricate corridors, finds himself in a common building, the first floors of which are empty.

Exploring the premises of the complex, Sven discovers barricaded doors leading to the main hall. He knocks on the door, but almost gets shot, managing to shrink to the side. Sven hides and tries to convince him that he is fine and an ordinary person. A man's voice outside the door demands proof of his humanity. Sven is given an ampoule of dark red serum without a needle and asked to drink its contents.
Sven is experiencing withdrawal, he is shaking. He greedily drinks the contents of the ampoule, but soon he feels bad and nauseous. Trying to catch his breath, he receives several bullets. A voice outside the door says that the serum was supposed to kill him. Sven faints from the pain and hears the man who shot him condemn him, repeating that the serum should have killed him by now.
Sven wakes up in the corner of the main lobby of the Northern Peleis Common Block. He notices a lot of people in the form of patients. One of them examines him and says that his wounds have already healed. Sven asks why all the survivors have gathered in the main hallway. The stranger replies that this happened on the orders of the head of the hospital, Gerhard Grakham, but he did not get in touch.
Sven meets Anna, a girl with an amazing regeneration. She immediately attracts him with her beauty, which seems perfect to him. Anna says that she also ended up here from block B8. She escaped from there during the procedure and the mass attack of the Changed.
Anna steps away from the others and reveals that she also got into this block by drinking an ampoule of dark red serum. She hints to Sven that she knows about his addiction to the serum. Victor says that he worked in the general archive of the institution and saw records of the symptoms of the virus codenamed "Retribution". He says that he got access to the recordings from an assistant to the head of Northern Peleys named Sshiru Koto.
Hearing this name, Sven remembers seeing a Shiru Koto patch on his deceased grandfather's uniform. He remembers his words about an illness that kills orderlies and changes patients. Victor says with regret that Sshiru died. He says that he had cirrhosis of the liver, and he dreamed of returning to his wife and children, who did not visit him. Victor says that Sshiru was happy when he found out that he had a healthy liver thanks to the orderlies.
Anna joins the conversation and says that additional organs were visible on her ultrasound. She says that her brother, with whom she grew up as orphans, never visited her. Victor says that Sshiru was glad that he now has a second healthy liver.
Victor says that Sven must find Sshira in the archive, where records of the symptoms of the virus are stored. Victor says that Sshiru was an aide to the head of Northern Pelais named Sshiru Koto. Victor regrets the news of Sshiru's death, remembering that he suffered from an advanced form of cirrhosis of the liver and dreamed of returning to his family, who had never visited him.
After Viktor's words, Sven notes the joy of the old man, who said that now he has a second healthy liver thanks to the efforts of the orderlies. Anna joins the conversation and says that the results of her ultrasound have repeatedly revealed various organs. Her brother, with whom she grew up without parents, also never visited her.
Suddenly, other patients join the conversation, admitting that they have also noticed additional organs while waiting for their loved ones who did not visit them. Sven, remembering the twins' mention of his wife, tries in vain to remember anything about her, wondering about her existence.
Over the next few days, the group attempts to reach the top of the ventilation shaft, but each time fails due to its strong and slippery walls. Through the lower ventilation shaft, Sven, as part of a small group, regularly makes forays into other blocks of the complex in search of supplies and medicines.
Fighting off the altered and various madmen, the group discovers a hiding girl in a dirty nurse uniform, whom Sven recognizes as Emmy's twin. In the office where the girl was found, Sven finds a list of the names of Gerhard Graham's deputies, in which one detail arouses suspicion - this position was held every three months by a new person.

Once in the main hall, the girl reveals that her sister simply disappeared a few days before the massacre began. In an attempt to support Emmy, Victor, Anna, and Sven promise to find her sister.
That evening, Sven visits Emmy again and tells her about a photo of herself standing next to her sister in the company of a man. Sven wonders if the man is Gerhard Graham, the head of the hospital. Emmy answers in the affirmative and assures that her father from childhood would definitely save everyone from this nightmare.
The girl admits that a few years ago, her father changed a lot after completing some research. He became a secretive, cynical and cruel person with sudden changes in mood. Soon after, Emmy's father goes missing, and his deputy, Lewis, does everything he can to hide and obstruct the investigation of his disappearance.
Despite the constant forays, the survivors are still short of food and other supplies. The situation is saved only by rare supply boxes that fall from the upper ventilation shaft along with doses of serum.
At night, Sven wakes up from another nightmare, in which the silhouette of a naked woman, consisting of organic threads, slowly stretches her hands towards a completely immobilized man. Suddenly, he is woken up by Anna, holding a box filled with ampoules of serum.
Sven tells Anna about the information found in the archive that serum addiction was induced artificially to suppress the virus cells in their bodies. Anna admits that she can't resist her withdrawal, and they both take all the vials from the box.
Experiencing an overdose, Sven and Anna feel a strong attraction to each other. At this time, something huge tries to break the barricade in the main hall, waking up the whole group. The barricade collapses, and a huge carcass, like a human heart, bursts into the hall, tearing apart everyone it meets.
At the same time, Sven and Anna have passionate sex in various positions, not hearing what is happening in the main hall due to an overdose. The carcass kills most of the group. Victor tries to get to Sven and Anna's room, but fails and is nearly killed by the mascara's attacks.
Suddenly, Sven passes out right during intimacy, leaving Anna perplexed. At the last moment, Victor manages to find Emmy and leave the room with her through the ventilation, hoping that Anna and Sven will survive. Noticing Victor and Emmy running away, the pursuer follows them. Trying to recover, Anna immediately senses something is wrong and runs naked into the main hall, filled with corpses and covered in blood. She feels sick from what she sees - she vomits, and she falls to the floor, losing consciousness.
Under the loud crying of a newborn, obstetricians take out a distorted silhouette, only remotely resembling a healthy baby. A voice from the darkness asks the orderlies to try a combination of the previous two serums next time, and to send the newborn to "get closer" with brothers and sisters. After that, a middle-aged man in a long jacket, whose face is hidden by a shadow, approaches the woman resting from a recent birth. The man congratulates her on a successful birth, revealing that she set a record for the number of children this week, making Rudy incredibly strong. In response, the woman asks if everything is okay with Lily. The man assures her that Rudy took care of his sister's safety by moving closer to the bed.
Turning on the light above the bed, which is the same psychotherapist that Sven visited, the man injects himself in the groin area and, presumably, has sex with the woman. After orgasm, the man is struck by a stroke, and his lifeless body falls to the ground in convulsions. The woman's abdomen swells sharply, and she begins to suffer from contractions. The body is carried away by orderlies, and the woman gives birth to a beautiful blue-eyed baby. The orderlies immediately take the baby away from the woman, leaving a scar on his abdomen when the umbilical cord is cut due to his mother's resistance.
