OBSERVATION:
Patient #307 is a male of stout build appearing to be in mid to late sixties. Well-nourished with above-average muscle mass for age. Unusual facial features and uncommonly greyed skin color make ethnicity impossible to guess. Grooming appears inadequate. Hair and beard are long and unkempt. Prominent, poorly healed scarring at the hairline above both eyebrows. 307 is currently supine in bed. Responds to verbal prompts. Able to spontaneously move all limbs, but motor action is delayed due to chemical sedation.

Little to no background information is available. 307 arrived on 8/25/2085 via ambulance, having been apprehended in the street during a violent, delusional outburst.
307 appears to be a local or to have at least spent some time here in Grünwald, as he has knowledge of the town’s history before its incorporation into San Antonio proper. In particular, 307 makes oblique references to figures of local tall tales and myth, such as the “mother bear,” which I will not delve into here. Suffice it to say this patient demonstrates a marked fixation on the inarguably tragic and arcane events of Grünwald’s history, both real and fictitious.
SUBJECTIVE:
307 is extremely somnolent due to chemical sedation, demonstrating none of the belligerent and paranoid behaviors that have been well-documented over the five days since initial admission. 307 normally becomes suspicious with inquiries about personal history, frequently attempting to redirect questioning. However, on this date 307 is unusually cooperative with questioning about the content of delusions, likely due to the efforts of the staff to provide a stable medication regimen.
OBJECTIVE:
All vital signs remained within normal limits throughout the session.
This session consisted of 75 minutes of one-on-one, face to face interaction. Initial interaction was pleasant, but confused. When I inquired if he was born here in Grünwald, 307 became insistent that he was not born in this world at all, but in another world in a different dimension. He stated he was glad to be human, but was not born as one.
In an attempt to assess the patient’s orientation to reality, visual perception, and fine motor coordination in his current state, I gave 307 a set of art supplies and requested a self-portrait. The result is attached below.

307 insisted that he and every other citizen of his original world was an animal of some kind. In an attempt to lead him to speak more on the subject, I made a provocative suggestion that sheep are not considered among the most intelligent of beasts. 307 grew significantly more alert and answered that the sheep of his world, along with the dogs, pigs, and birds, were able to think and speak just like humans. Taking advantage of his increased responsiveness, I requested more details about the patient’s life in this world of his. He launched into a narrative about his previous life prior to his “travel to this dimension.” I have captured this narrative on a series of tapes which remain in my possession.
I will present my findings in sequence as I finish transcribing each audio recording.
